Doctor Discussion Guide

The Doctor Discussion Guide12 is a convenient tool to help you prepare for a discussion with your healthcare professional (HCP).

Are you meeting the demands life places on you? Now that you know you have ADHD—and that effective treatments are available—what is your goal? To achieve your potential at work and at home? Are you reaching it?

This interactive questionnaire can help you learn more. Only you and your doctor can determine the right treatment plan for you. But by answering a few questions, you may be able to tell whether your current treatment is working. Are you the person you know you can be-without ADHD standing in your way?

Instructions: For each statement below, please select the response that best applies to your behavior or feelings over the past 6 months. Then hit the submit button to see your results. You can clear your answers and start over by clicking on the reset button.

When you have finished answering the questions, simply print the PDF file and take it with you to review with your doctor on your next doctor visit.
