The ADHD Doctor Discussion Guide is a convenient tool to help you prepare for a talk with your doctor.

The guide contains an interactive questionnaire to help you and your doctor talk about any changes in your behavior that may be related to your ADHD symptoms.

When you have finished answering the questions, simply print the PDF file and take it with you to review with your doctor at your next visit.
